- If you are seeking an in clinic appointment: search locations here, or you may request or schedule an appointment online through OSF MyChart 24/7. Please note that urgent care clinics are not open later than 8pm. See the location list for further detail.

- If you are having issues logging into your account, need help with RX, or encountered an error while completing a visit, chat using the icon in the lower-right corner of the screen, or call (309) 248-0660. Support center hours of operation are Monday-Friday from 6AM-10PM CST and Saturdays, Sundays and Holidays 8AM-8PM CST. If you encounter an issue after hours, feel free to leave a voicemail and the support center will follow-up with you promptly during business hours.
